The patient who has a medical prescription for a PET-CAT scan can download the appointment form on this page in the box called “NECESSARY FORMS”, otherwise can pick it up at the Gemelli PET-CAT scan Center (located at the CEMI Multifuncional Building Complex that is within the Policlinico Gemelli area), or have it sent by fax. Once the form is filled out, the patient must personally bring it back to the Center, otherwise send it via fax (fax number 06 3013.745) or send it as an e-mail attachment (pet-tac@policlinicogemelli.it)
The medical report can be picked up Monday to Friday from 10:00 am to 7:00 pm. When picking up the report it is necessary to show the receipt given upon registering and a personal document.
The Center’s specialists, Nuclear and Radiology Doctors, will evaluate the request’s appropriateness and will see to establish if there is a medical urgency. The Center’s administration office will then contact the patient (or the department/the external structure) at the numbers that the patient provided in the appointment form in order to be informed of the PET-CAT scan appointment date and the relevant instructions.
